Bilateral idiopathic localised involutional lipoatrophy in a child

A previously healthy 7-year-old girl presented with a 2-month history of an oval lesion in the left calf and two smaller lesions contralaterally, which were expanding (figure 1). Physical examination showed the presence of an asymptomatic skin depression measuring about 6x4cm in diameter with a hypopigmented and atrophic surface without inflammation. Two similar lesions, both 3.5x2.5 cm in diameter, were contralaterally noted. No history of injury, injection of drugs or preceding signs of inflammation were reported. Blood tests including autoantibody tests were negative. On biopsy, the histology showed an unremarkable change in the epidermis and dermis without inflammation. Variable-sized fat lobules composed of small cells (resembling fetal fat tissue) in the background of well-vascularised tissue with capillary hyperplasia were observed (figure 2).
