We read with great interest the article titled "Transient oculomotor nerve palsy after topical administration of intracisternal papaverine" by Chittiboina P, Willet O, Nanda A, Guthikonda B. Acta Neurochir DOI 10.1007/ S00701-010-0849-5 [2]. Intracisternal instillation of papaverine in patients at risk of developing vasospasm is routinely followed worldwide as well as at our institution after successful surgical obliteration of ruptured saccular aneurysm by clip application. We too have made similar observations as done by the authors. Pupillary fibers are almost always involved, and a few of our patients have also developed transient oculomotor nerve palsy (TONP), either partial or complete.
