Endovascular treatment of internal carotid-posterior communicating artery wide-necked aneurysm using 2 Enterprise stents in Y-configuration

Endovascular treatment for intracranial aneurysm is now widely accepted, but wide-necked aneurysms remain a challenge to the interventionist. When wide-necked aneurysms arise from a vessel bifurcation site, such as basilar top aneurysms, a second stent can be advanced through the struts of the first stent into the second side branch, where the 2 stents form a Y-configuration. This so-called Y-configuration stenting technique can be useful to secure the aneurysm with preservation of luminal patency of the parent vessels at the basilar top artery, middle cerebral artery, and anterior communicating artery [1,2]. We hereby present a case of a wide-necked aneurysm arising from the internal carotid-posterior communicating (IC-Pcom) artery, which was managed by endovascular treatment using 2 Enterprise stents placed in a Y-configuration and subsequent coil embolization. To our knowledge, this is the first report of Y-configuration stenting and coiling at a ruptured IC-Pcom artery aneurysm.
