PHARMACIST-LED CLINIC MAY REDUCE HOSPITAL READMISSIONS

Patients attending a pharmacist-led medicines optimisation clinic (MOO) after being discharged from hospital were less likely to have been readmitted after 180 days, a study has found. Researchers studied 62 patients who were considered to be at high risk of medicine-related problems after hospitalisation. Half of patients attended a MOC with a hospital-based clinical pharmacist, while the other half received normal post-discharge care.
