High dose statins should be used in all patients undergoing percutaneous coronary intervention

I have read with great interest the article entitled 'Comparison of high reloading ROsuvastatin and Atorvastatin pretreatment in patients undergoing elective PCI to reduce the incidence of MyocArdial periprocedural necrosis. The ROMA II trial' by Sardella et al. in the online issue of the journal [1]. They have demonstrated that high dose statin reload significantly prevents periprocedural myocardial injury and major cardiac and cerebrovascular events up to 1 year. They have mentioned that they demonstrated, for the first time, the efficacy of pretreatment high dose (40 mg) rosuvastatin in preventing periprocedural myocardial injury in 80 patients with stable coronary artery disease undergoing elective percutaneous coronary intervention (PCI) in their previous ROMA trial [2]. However, we already demonstrated that a single high loading dose (40 mg) of rosuvastatin before the PCI procedure in 153 statin naive patients reduced the incidence of periprocedural myocardial necrosis and infarction effectively.
