We need a 'Pharm-2-Pharm' application

I write regarding Andrew McCoig's letter about transfer of care (PJ, 23 June 2012, p738). My trust was an early adopter site of the Royal Pharmaceutical Society transfer-of-care guidance. Our aim was to implement some of the guidance and we chose to improve the quality of information sent back to GPs. Advanced community pharmacy services only started four months after the guidance was published. However, 12 early adopter sites chose to include ways of improving communications between hospital and community pharmacies.
