A bright future for better transfer of care: an example from one NHS trust

SO KEEN was the lead pharmacist for the East Lancashire Hospitals NHS Trust (ELHT) to be an early adopter of new transfer-of-care guidance from the Royal Pharmaceutical Society (PJ, 9 July 2011, p57), that he instantly drafted a list of 15 action points to tackle the issue throughout his trust. In the two months since the scheme was introduced, the ELHT has achieved all but one of those objectives, including a comprehensive revamp of its electronic discharge prescriptions.
