Chief pharmacists have been called on immediately to ensure all vaccines are stored safely, following a series of incidents relating to their incorrect storage. The recall of 560 patients from two GP practices for repeat vaccination following incorrect vaccine storage, along with 260 reports of related incidents over the past four years, has prompted the National Patient Safety Agency to issue a rapid response report on the issue.
