Folk acid fortification, instituted in the mid to late 1990s in the USA and Canada, and now in more than 50 countries, has been highly effective for its intended purpose-to reduce the incidence of neural tube defects (eg, spina bifida and anencephaly). However, lingering concerns about the safety of excess intake of folic acid still exist, particularly with respect to cancer.5
