In January, 2009, a 40-year-old woman presented to us with a left-sided Homer syndrome and headache. MRI showed a wall haematoma of the left internal carotid artery, which was consistent with internal carotid artery dissection (figure A). When resuming her running exercises a few weeks later, she noticed right-sided facial flushing and sweating during physical exertion. The left side of her face remained pale and did not sweat (figure B). These symptoms resolved within a few minutes of rest.
