Extramammary Paget's disease (EMPD) of the penis and scrotum is a rare genitourinary malignancy primarily affecting patients 60 to 70 years old. Patients present with pruritus and pain associated with an erythematous, eczematous or ulcerated plaque-like lesion with well-defined borders. The most frequent location of EMPD in males is the penoscrotal junction, followed by lesions of the scrotum and inguinal area (fig. I). There is often a delay in diagnosis of EMPD until therapies for more common diseases including psoriasis, eczematous dermatitis and fungal infections fail.
