Successful treatment of porokeratosis of Mibelli with imiquimod 5% cream.

Porokeratosis is a well-recognised disorder of keratinisation initially described independently by Mibelli and Respighi in 1893. Several clinical variants have been recognised based on different distribution patterns. In porokeratosis of Mibelli, the lesion develops as a small, light-brown, keratotic papule that slowly expands to form an irregularly shaped annular plaque with a raised, ridge-like border. Malignant transformation is a known complication and has been reported as high as 7.5% in a case series of 281 patients of porokeratosis. Therefore, it is advisable to treat these lesions. Various treatments including keratolytics, topical 5-fluorouracil (5FU), etretinate, CO_2 laser, cry-otherapy, electrocautery and surgical excision have been tried. Surgical excision is the most definitive approach but this can be technically difficult in some cases, depending on the site, size and number of lesions.
