Combined use of cardiac m-i123-iodobenzylguanidine scintigraphy and (1)(2)(3)I-fp-cit single photon emission computed tomography in older adults with rapid eye movement sleep behavior disorder.

To the Editor: Rapid eye movement (REM) sleep behavior disorder can be idiopathic (iRBD) without concomitant neurological disorders or be associated with neurological diseases such as a-synucleinopathies, tauopathy, and narcolepsy. Evidence has shown that patients with iRBD may have damaged cardiac sympathetic nerve terminals, suggesting that this disorder may belong to Lewy body diseases. Here two patients with iRBD who showed cardiac sympathetic denervation on m-I123-iodobenzylguanidine (~(123)I-MIBG) scintigraphy but an intact nigrostriatal do-paminergic system on dopamine transporter single photon emission computed tomography (DAT-SPECT) scan are described. The diagnosis of iRBD was made according to the typical history and confirmed using video polysomno-graphy (video-PSG) in the absence of other neurological diseases. DAT-SPECT and cardiac MIBG scintigraphies were performed according to our routine protocol. Values were considered abnormal when they were more than 2 standard deviations below the control mean.
