Improving performance in single-incision laparoscopy.

I read with great interest the study by Rieder and colleagues1 concerning the use of a triangulating surgical platform in single-incision laparoscopy. As an "average Joe" general surgeon practicing in a medium-sized suburban community, I decided to become an early adopter of single-incision laparoscopic surgery (SILS) in 2009 because I thought it would add a challenging and aesthetically enhancing aspect to laparoscopic procedures that I have been performing for almost 20 years. Since 2010, I have performed more than 20 SILS inguinal hernias, more than 25 SILS cholecystectomies (5 with cholangiograms), more than 20 SILS appendectomies, and a spattering of SILS colectomies, ostomies, and ventral herniorrhaphies.
