Temtosecond laser technology has advanced the field of refractive and corneal surgery. In keratoplasty, the ability to create a variety of corneal incisions precisely has allowed surgeons to closely match the shape of donor and recipient corneas, which has decreased healing times and lowered astigmatism when compared to conventional corneal transplantation.The most common configurations used for femtosecond keratoplasty are the zigzag, top hat, and mushroom shapes. Vertical side-cut trephination can be performed with the femtosecond laser as well. However, the more complex configurations have the advantage over the vertical side-cut in that they increase the surface area of the graft-host interface. This may improve the stability of the transplant wound, which has been demonstrated to be at risk of dehiscence even many years after surgery.
