Severe epithelial keratopathy after hyperopic presbyopic photorefractive keratectomy.

Punctuate epithelial keratppathy is a common complication after excimer laser ablation. However, recently, one of our patients developed severe bilateral epithelial keratopathy after hyperopic presbyopia-correcting (multi-zone ablation) photorefractive keratectomy. The patient was a 56-year-old woman. Manifest refraction in the right eye was +2.25 -1.00 X 90 and + 2.50 -1.50 X 90 in the left eye. Distant and near correction (add +2.25 in both eyes) resulted in 20/20 vision in both eyes. She had no history of ophthalmic disease. Slit-lamp examination revealed bilateral normal corneas with no sign of limbal insufficiency. Normal corneal topography (Pentacam; Oculus Optikgerate, Wetzlar, Germany), tear break-up time (10 seconds both eyes), Schirmer test (10 mm right eye, 12 mm left eye), and corneal sensation (6 mm both eyes, measured by Cochet-Bonnet esthesiometry) were observed. Intraocular pressure was 12 mmHg in both eyes.
