Psychiatric nurses often note that we suffer from second-class status professionally and financially (Ward & Cowman 2007, Hanrahan & Aiken 2008, White & Kudless 2008). Around the world a variety of healthcare finance mechanisms are in place that put healthcare providers in the roles of insurers. Whether these are called 'profit sharing', 'risk sharing', 'managed care', 'capitation contracting', 'Diagnosis related groups' or simply annual budgets, the impact is often the same. In the United States the 'gorilla in the room' of healthcare (finance) system reform is the deeply flawed 'capitation' mechanism.
