A 39-year-old man was referred to our institution for a history of relapsing alcoholic pancreatitis. He had six episodes of severe acute pancreatitis in the past few years. He had stopped drinking one year and a half before but remained symptomatic. Magnetic resonance (MR) imaging performed for preop-erative evaluation showed diffuse pancreatic atrophy with ditated main pancreatic duct, which measured 5 mm of maximal diameter (Fig. 1a). A mass of intermediate signal intensity on T1 and T2-weighted images was seen within the gastric antrum, corresponding to heterotopic pancreatic tissue. Some cystic formations were seen within this mass and within the duodenal wall at the pancreaticoduodenal groove (Fig. 1a and b), demonstrating high signal intensity on T2-weighted images. These cysts showed peripheral rim enhancement following gadolinium injection.
