A 29-year-old trauma patient underwent operation of revascular-ization of the injured left popliteal artery and open reduction internal fixation for the left femoral fractures. Subsequently, the patient developed severe pain in the distal left lower extremity. A CT angiogram revealed patent three-vessels of his left lower extremity. However, an FDG PET/CT demonstrated absence of FDG uptake in the left foot, which suggested soft-tissue nonvi-ability. An amputation was performed the following day. The pathologic examination showed wide-spread necrosis of the entire left foot with extensive dry gangrene, which confirmed FDG PET/CT findings.
