High extracardiac activity connected to the myocardium in cardiac single photon emission computed tomography (SPECT) frequently interferes with the reader's interpretation confidence as a result to reduce the diagnostic accuracy. Limitations are recognized as low spatial resolution of SPECT with parallel-hole collimation and large scatter component from liver or bowel that mainly affects interpretation for defects in the inferior wall. Cardiac positron emission tomography (PET) with higher spatial resolution and more robust scatter correction clinically is an alternative for nuclear myocardial perfusion test and can be further considered as the solution for the patient with a difficult SPECT study. In this article, we demonstrate the role of PET by directly comparing SPECT images with PET images on the same patient.
