The presence of myocardial scar and/or myocardial ischemia provides an important substrate for the occurrence of potentially fatal ventricular arrhythmias. A 74-year-old man developed extensive ischemia associated with cardiac arrest during exercise stress myocardial perfusion scintigraphy. Technetium-labeled perfusion tracers are trapped inside myocardial mitochondria, and therefore show regional differences in coronary flow at the time of tracer administration. SPECT images acquired after resuscitation from cardiac arrest demonstrate the ischemic myocardium associated with his cardiac arrest.
