The measurement of troponins in blood has rapidly become an accepted method for detecting myocardial damage. Furthermore, recently redefined criteria for myocardial infarction used to classify patients with acute coronary syndrome have been established on the basis of increased serum/plasma concentrations of cardiac troponin I (cTnl) or T (cTnT). Studies have indicated the prognostic importance of increased troponins in various clinical settings. In fact, incorrect quantification of cTnl may occur due to antibody and non-antibody interferences. The presence of interfering antibodies such as heterophilic antibodies and rheumatoid factors can affect immunoassays and produce either falsely elevated or falsely low results. In addition to these factors, non-antibody interferences such as residual fibrin and microparticles may also be a contributing factor to incorrect measurement of cTnl.
