A 29-year-old medically healthy woman was referred for "cosmetic" cataract extraction in the left eye. She has had no vision in that eye since birth and for many years has been bothered by its appearance. The pupil aperture contains a yellow-white sub-luxated mature cataract and the corneal diameter is smaller than that in the fellow normal right eye. She has had varying degrees of success with a custom cosmetic contact lens but now finds the lens too uncomfortable to wear. Conversation with the patient regarding her options included the possibility of enucleation, which she refused to consider. Other than the difficulty with contact lens wear, the eye causes no discomfort.
