Rupture of the posterior lens capsule during cataract surgery is the most frequently encountered serious intraoperative complication, occurring in 1.5% to 2.0% of cases.1"3 It may preclude placement of an intraocular lens (IOL) in the capsular bag, but an alternative is implanting the IOL in the ciliary sulcus. To avoid inducing myopia, many surgeons reduce the power of the IOL, although there is little clinical evidence to guide this. Ophthalmologists may therefore adopt an empirical approach or refer to available guidelines.
