Endothelial keratoplasty is the preferred treatment for endothelial dysfunction in the United States.1 Descemet-stripping automated endothelial keratoplasty (known as DSEK or DSAEK) is the most widely used method, but a growing number of surgeons are transitioning to Descemet membrane endothelial keratoplasty (DMEK). The key difference between DMEK and DSEK is the inclusion of supporting donor stromal tissue. The supporting tissue in DSEK facilitates handling for surgeons and improves adhesion at the expense of a larger incision size, increased higher order aberrations, and a higher rejection rate in patients relative to DMEK.
