Reply: Laser in situ keratomileusis after deep anterior lamellar keratoplasty

We would like Acar et al. to clarify some points in their recent study.1 In the discussion, it is stated that "[c]ustom ablation was not used because most cases had a regular astigmatism profile." Eyes with keratoco-nus and penetrating keratoplasty have higher-order aberrations (HOAs) that are approximately 5.5 times greater than the typical aberrations in normal eyes.2 Dramatic visual improvement can be expected by correcting the HOAs in these eyes.
