Ventricular tachycardia ablation: what have we learned from SMASH-VT.

In the current issue of the Journal, the SMASH-VT investigators analyze the clinical and procedural predictors of outcomes of ventricular tachycardia ablation.To put this paper in context, we need to review the results of the SMASH-VT trial (Substrate Mapping and Ablation in Sinus Rhythm to Halt Ventricular Tachycardia).SMASH-VT was a small study of 128 patients with a history of a myocardial infarction 1 month or longer prior to enrollment with ECG or cardiac imaging documentation and had undergone ICD implantation for ventricular fibrillation, hemodynamically unstable ventricular tachycardia or syncope with inducible ventricular tachycardia. Later, enrollment was extended to include patients who received an ICD for primary prevention and received a single episode of appropriate ICD therapy.
