THE UNITED STATES-BASED Institute of Medicine (IOM) re-ported in 1999 that "at least 44,000 people, and perhaps as manyas 98,000 people, die in hospitals each year as a result of medicalerrors that could have been prevented, according to estimatesfrom two major studies." A new study published in 2013 by theJournal of Patient Safety states that four times as many peopledie from preventable medical errors than originally thought-asmany as 440,000 a year.
