In an effort to help curb the nation's soaring rates of overweight and obesity, the American Academy of Family Physicians' (AAFP) Americans in Motion-Healthy Interventions (AIM-HI) program has developed an online nutrition questionnaire to help patients assess their risk of nutrient deficiencies and to help family physicians strategically address the nutritional habits of patients and their families.With patients increasingly seeking health information online, this free resource is designed to motivate patients to practice healthy eating behavior. The free online tool is designed to encourage patients to work with their family physician to assess potential risk for nutrient deficiencies that may contribute to health problems. The tool asks how often the patient eats various types of fruits, vegetables, dairy products, fish, and meat, as well as how many fresh and processed foods they purchase.
