Intensive blood sugar control in patients with Type 1 diabetes mellitus is expected to delay progression of atherosclerotic disease. The authors evaluated the changes in carotid intima-media thickness (CIMT) after pancreatic islet cell transplantation in a case series of 15 patients with Type 1 diabetes mellitus. All patients achieved insulin independence after one to three transplants. At 12 months post-transplantation CIMT for the common carotids decreased significantly in all patients. There was a continued reduction in CIMT at long-term follow-up. Islet cell transplant restores euglycemia and ameliorates diabetes-related atherosclerosis. This study was limited by the small patient numbers and a lack of a control group.
