QUEENSLAND MING AND QUARRYING INDUSTRY SUMMARY OF SERIOUS ACCIDENT AND HIGH POTENTIAL INCIDENT REPORTS FOR THE TWO WEEKS ENDING 2301/2005

A flameproof cover containing the pilot circuit to a shearer was left loosely secured in place after the completion of testing. A shearer failed to stop when the tram button was released. Other control functions were tried but the shearer didn't stop until the trip wire was activated. An Eimco 913 front end loader ran backwards down a 1 in 4 gradient drift for 200 metres when the operator lost control on a section of the drift that was wet and slippery. A loaded Cat 789 rear dump truck, after entering the main haul road from a ramp, veered off the road into the table drain and toppled onto its side. The operator of a rear dump truck reversed to the tip face on an angle instead of perpendicular to the face. The left hand rear wheels impacted with the safety berm causing it and the tip edge to slump and the wheel to bog. A Cat D11 dozer was working directly behind a dragline and within the swing radius of the machine counter weight. The right hand rear corner of the dragline hit the dozer causing extensive damage to the dozer lift ram, fuel tank, hydraulic tank and access platform.
