On an unseasonably warm June 1 in 2011, Colorado Gov, John Hickenlooper left his office in the state Capitol and drove a few miles west to Lakewood, a Denver suburb. Arriving at St. Anthony Hospital, he sat down at a small table and, with the minimal formality of a man who almost never wears a necktie, signed a bipartisan bill creating a state health insurance exchange.
