Diabetes UK has produced a pocket-sized information leaflet, supported by Aventis, with spaces for patients with diabetes to record measurements of HbA~lc, blood pressure, blood lipids, waist circumference and body mass index. Copies of the card have been sent to Diabetes UK members and to doctors' surgeries.
