The reconstruction of mandibular segmental defects follows well-established biomechanical principles in relation to the defect site and size, the quality of surrounding soft tissues, and the patient's general health status.Although the restoration of extensive mandibular defects with vascularized bone represents the gold standard, the use of reconstruction system plates is a feasible option to bridge lateral mandibular defects when the condylar head is preserved. Conventional load-bearing reconstruction systems are usually preferred in poor health status patients or in the presence of specific contraindications to autologous bone flap harvest. Reasonable and stable functional results can be achieved provided that enough bone stability is obtained.Several reconstruction plate systems exist that require different degrees of plate adaptation to bone, but all must accomplish to the general principles of preplat-ing and bicortical screw fixation.2'11'12'14 Relatively frequent complications are observed following the insertion of reconstruction plates, with plate/screws fracture being the most common.
