Drug-induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms syndrome induced by cilostazol and carbamazepine

A 54-year-old man presented with a fever and a maculopapular rash on his trunk, extremities and face. At the time of admission, he was taking an inhaled corticosteroids/long-acting p-agonist combination for bronchial asthma, carbamazepine (CBZ) for peripheral neuropathy (for 2 weeks), and triple antiplatelet therapy (cilostazol, aspirin and clopidogrel) due to coronary stent placement approximately 3 months prior.One week before admission, he developed fever, myalgia and skin eruptions. At admission, his temperature was 38.0°C, and he had a macuiopapular rash on his trunk and extremities. Exfoliative dermatitis also observed on his face. Laboratory tests revealed a leu-kocytosis (27 060/mm~3) with eosinophilia (1802/mm~3).
