Family nurse practitioner (FNP) residencies and fellowships are a new strategy for hiring and retaining skilled primary care providers and are supported by the Institute of Medicine. FNPs have been shown to benefit from increased support during their first year of practice. Margaret Flinter outlined a model of a 1-year FNP residency in a federally qualified health center with intensive orientations, assigned preceptors, and a gradual assumption of responsibilities. This model has been used to improve patient access, provider diversity, and provider retention. Three FNP residents can generate enough revenue to offset the nonproductive preceptor time.
