Randomized controlled trial of ultrasonic dissection versus standard surgical technique in open left hemicolectomy or total gastrectomy (Br J Surg 2011; 98: 220-227).

We read with interest the recent randomized controlled trial by Wilhelm and colleagues on ultrasonic dissection verms standard surgical technique in open surgery. However, the article omits important information on the standardization of the two chosen treatments (left hemicolectomy and total gastrectomy). The patients were recruited from three centres in two different countries and were operated on by an unknown number of surgeons, with inevitably variable postoperative regimens. Specifically, use of an enhanced recovery protocol and the availability or reason why laparoscopic surgery was not undertaken is not mentioned. Despite this, a significant proportion of the results section analyses factors such as postoperative pain and return of bowel movements.
