The use of minimally invasive transanal techniques for the surgical treatment of significant rectal neoplasms poses several challenges. The various 'crossover' techniques such as that described here all have merits in reducing costs, for both capital equipment and consumables, as well as addressing the physical restrictions of the transanal endoscopic microsurgery (TEMS) proctoscope, instrument ergonomics and patient positioning, which demand experience and expertise to overcome, gained mostly through larger case series. Tapping into the familiarity of equipment found among laparoscopically adroit surgeons is a rich vein and undoubtedly offers potential benefits of improved 'instrument freedom' and 'ready to go' skills that are attractive.
