Hydatidosis still accounts for an endemic health problem in the Middle East, Australia and South America, with 50% to 70% of hydatid cysts occurring in the liver and 20% to 30% in the lung. Due to the growing immigration, the prevalence of hydatid disease in North America and Europe is increasing. A 25-year-old woman was referred because of a 3-month history of nausea, vomiting and early satiety. She was from a rural region in northern Iran. Ultrasonography revealed multiple cysts in the liver. Laboratory tests revealed positive anti-Echinococcus granulosus antibody and eosinophilia in a peripheral blood sample. An abdominal computed tomographic scan revealed 9 hypo-dense masses (Figure 1), with internal septation suggesting hydatid cysts. Pulmonary evaluations were normal. The patient received albendazole 10 mg/kg/d for 2 weeks and then underwent surgery. Nine large cysts involving all segments of the liver were found. Drainage and endocystectomy were performed starting with superficial cysts. Deeper cysts were reached by piercing inner parts of drained cysts. Finally, 9 cysts were coalesced into 2 large cavities, drained by 2 multihole tube drains.
