We read the article with great interest and congratulate the authors for their outstanding feat on performing natural orifice transluminal endoscopic radical prostatectomy with vesicourethral anastomosis. However, we would like the authors to address 2 issues. First, 1 of the important steps in radical prostatectomy is to perform a tension-free vesicourethral anastomosis to avoid bladder neck contracture. Although the authors performed the anastomosis transurethrally, we believe that the wide gap created after excision of the prostate would prevent a tension-free anastomosis. How do the authors propose to overcome this problem for larger prostate glands?
