The peer review process for the recent article in Urology by Featherstone et al raised many interesting points, one of which concerned the relevance of the drug distribution results presented with regard to clinical tumors and potential treatment with multidrug resistance-reversing agents. In response, an allusion appears in the text of the article to a small unpublished series of primary explant cultures from clinical material in which reversal of resistance in terms of nuclear anthracycline accumulation was observed by cotreatment with both verapamil and PSC833-a cyclosporin analog that modulates multidrug resistance independent of P-glycoprotein. To incorporate this series in the article by Featherstone et al would have been impractical, but we take the opportunity to present a composite figure here.
