Over the last decade or so, as a steady stream of high-profile accidents have occurred and regulations have tightened, the term "human element" has come into wide usage within the marine industry. Within the context of accident or casualty investigations, it's generally used to distinguish those deemed to have been caused (in whole or in part) by human failings of shipboard crews from those caused by something else, typically the failure of a piece of equipment or propulsion, steering, navigation electronics, or the like.
