One of the roles of CROSS is to look for adverse trends and issue warnings when they are found. To paraphrase the words of Auric Goldfinger in Ian Fleming's novel1; "Once is happenstance. Twice is coincidence. Three times is concern about structural safety." Many reports are one-off incidents of failure or poor practice but others, looked at over time, show similar features and these are often the ones which are a real worry. If there is a basic defect in a component which is going to be replicated many times, there is the potential for many failures.
