First Great Western has begun an investigation after the 11.01 Penzance-Paddington service failed between Pewsey and Savernake on August 4. The failure happened after the brakes locked on due to a fractured air pipe. The power cars were Nos. 43150 and 43152. At the time of the incident there were 500 passengers on board. Although the driver made attempts to locate and fix the fault, he requested a fitter, who was delayed as he was fixing another failed HST at Reading. Class 59 No. 59202 was despatched to wait in the Woodborough loop in case it was needed. When the fitter did arrive, at about 18.00, the fractured pipe was somewhere under the rear power car 43150, so this was detached. No. 59202 was then used to drag it back to Westbury, leaving the solitary power car 43152 to work the train forward to Paddington, arriving at 22.15, six hours late.
