A spad (signal passed at danger) incident at Stafford on April 26 was caused by a combination of a speeding loco and a faulty speedometer, the Rail Accident Investigation Bureau (RAIB) has ruled. The incident involved a Riviera Trains Class 47, on hire to Devon & Cornwall Railways, which is owned by British American Railway Services (BARS), and driven by a person who had been hired for the journey. The SPAD occurred at signal SD4-81 (Stafford No. 4 box), which is located on the down slow line, south of the station. It controls entry into the station and protects train movements traversing Stafford South Jet. The locomotive passed the signal at about 30mph and came to a stand about 80 metres beyond. RAIB said the incident was included in general information provided by the railway industry and that it was not until September 2012 that RAIB was notified of the full circumstances leading up to the incident.
