This article presents the case of a 71-year-old female patient with eczematous, therapy refractory skin alterations of both nipples over a period of 2.5 years. Despite intensive gynecological and radiological investigations no abnormalities could be determined. The operative treatment of the bilateral Paget’s disease diagnosed by biopsy confirmed the presence of ductal carcinoma in situ (DCIS). The indications for biopsy should be generously interpreted particularly in cases of therapy refractory dermatological alterations of the nipples. If the suspected diagnosis of Paget’s disease is confirmed, operative treatment is indispensable even for exclusion of a causal ductal carcinoma.
