IMPROVING HEALTH CARE TRANSITIONS: TARGETING RESOURCES TO VULNERABLE ELDERS

Part of a ten-site study called Improving Health Care Transitions replication project, funded by the California HealthCare Foundation, this study, uses administrative data from a private non-profit hospital in Los Angeles County to identify patient characteristics that increase the risk of an avoidable hospital readmission.Among patients aged 65 and older, discharged in 2007 (n= 7,292), 25.1% were readmitted within 12 months.Over one third of those re-hospitalized had three or more admissions to the same hospital within the year.Using Cox regression, this hazard analysis looks at individual variables from electronic medical records to predict avoidable hospital readmissions.
