The Cheeki Rafiki tragedy: hull matrix failure or keel bolt failure?

At the beginning of the year, Ship & Boat International published an article, "Keel bolt integrity failure: the hidden killer", written by surveyor and FRINA Captain Ed Geary (Ship & Boat International January/ February 2015, pp15-17), in which the author advanced his sincere belief that keel bolt failure, and inadequate preventative action on the part of surveyors, led to the May 2014 break-up of the yacht Cheeki Rafiki and the subsequent deaths of her four crew members.
