Even the best-run ships can be caught out by a minor operational oversight or a sudden equipment failure. Normally, systems are in place to limit the damage, but on some vessels those systems are allowed to deteriorate to the extent that the vessel is primed for disaster. In a ship of this kind, an accident can quickly lead to a multiplicity of problems affecting the whole vessel.
