Australia's transport safety investigator has identified several safety issues at Patrick's Port Botany terminal in connection with a man's death two years ago. The stevedore lashing team leader was working aboard the box ship Vega Gotland on 28March2010 when he was crushed to death between two containers during loading.The Australian Transport Safety Bureau said in its report released at the end of December 2011 that the man had "positioned himself where he was exposed to the risk of being trapped", adding that he "had become desensitised to the risks associated with loading and lashing containers, and this resulted in him placing himself in a position of danger at the after end of the container being loaded".
